Health Services

If your health fails, it can overshadow everything else that’s going on in your life.
As a parent, it is important that you set a positive example. Parents and individuals should always be thinking about how they can improve their health and the overall health and wellness of their families.
Here at the Great Expectations Center we want everyone to be healthy and have access to health screenings and health care services. While we are not a medical facility, we do offer the following to services to support the health of our clients:
  • Free Pregnancy Testing
  • Free HIV Testing
  • Teen Pregnancy Prevention information
  • Disease Management Information (i.e. diabetes, blood pressure)
  • Breast /Prostate/ Colon Cancer information
  • Health and Nutrition information
  • Mental Health- Individual, family counseling
  • Cooking Classes
  • Non-diagnostic Sonograms (when available)
  • Assistance with obtaining Medicaid or Insurance
  • Health Care Referrals

Case Management

You are probably wondering what is case management. You may have heard about it, but thinking just another person I have to deal with or that’s asking me a lot of personal questions. Do I really need those services?
Our case managers are here for you! Their services are centered on the client and their support systems. Case managers are here to offer support in all areas that affects your daily life (e.g., medical, physical, functional, emotional, financial, psychosocial, behavioral, spiritual, and other needs), as well as those of your support systems. So, in essence, they help you, your family and or friends if that’s your support system.
These are the services case managers perform:
  • Evaluate what your needs are by asking questions and completing paperwork
  • Assess knowledge of resources, skill sets, and level of needs
  • Develop a plan of action or individual care plan based upon what you both determined your needs were
  • Assist with implementing your individual care plan and immediately identifying community resources
  • Give out referrals as needed for other services and support
